PARENT SUPPORT

Our staff depends on cooperation between the home and school to effectively administer health-related programs. Your assistance is vital in the following areas:
 
1. Emergency cards, forms, questionnaires, etc., need to be completed and returned promptly.
 
2. Changes in a telephone number, address, emergency contact, employment, etc., should be reported, in writing, to the school nurse and school office.
 
3. Information on a new immunization, surgery, accident, communicable disease, or special medical condition should be reported, in writing, to the school nurse.
 
4. Please keep students out of school if they experience a temperature above 100 degrees, vomiting or diarrhea within the 24-hour period.
