Kindergarten students must be five (5) years of age on or before August 31, 2025.

First grade students must be six (6) years of age on or before August 31, 2025.

To use the online registration feature in Skyward, the parent/guardian must have an email address.

• To create a Skyward account (new to Centennial School District): Create an Account

Once your register, you will get a follow-up email from the District Registrar.  That email will explain the next steps in the process, emails are sent daily.  If at any time you have questions or concerns, please contact the Registrar at 215-441-6000 ext 11046, email csdregistration@centennialsd.org, or click here to email CSD Registration.

After the online portion of registration is complete, you will be required to submit information & documentation for each of the five (5) categories explained below.

1. Proof of the Student’s Age – acceptable documentation includes one (1) of the following:

   • birth certificate;

   • baptismal certificate;

   • transcript of the record of baptism duly certified and showing the date of birth;

   • notarized statement from the parents/guardians indicating date of birth;

   • a valid passport;

   • or a prior school record indicating the date of birth.

2. Immunization Record (subject to review by District Nurse for compliance) with dates or assurance from the former school district or a medical office that the required immunizations have been completed or a required series has begun, with a record to be sent. Written statements are required for religious and medical exemptions.

3. Proof of Residency (current within 60 days of registration & in the guardian's name) – Four (4) total forms are required per the Residency Checklist.

   • One form must be a property deed/mortgage or current rental/lease agreement.

   • Three (3) additional proofs of residency from the following list are acceptable:

     • property tax bill,

     • homeowner’s insurance,

     • driver’s license,

     • current vehicle registration card,

     • current electric bill,

     • current TV/cable bill,

     • current utility bill,

     • current bank statement,

     • current credit card bill,

     • current paycheck stub with name and address,

     • welfare documentation with name and address,

     • or voter registration card with address.

4. Parental Registration Statement attesting to whether the student has been suspended or expelled for offenses involving drugs or alcohol, weapons, violence or conviction or adjudication for sexual assault against a student in the same school, as required by the Pennsylvania School Code.

5. Other Necessary Documents:

   • Updated report of physical exam and dental exam

   • Emergency contact phone number (2 required)-Click here for the required Fillable Emergency Card

   • Prior school’s report card/transcript/attendance information

   • Legal Custody Order (if applicable)

   • Individualized Education Program (IEP) or other special education records (if applicable)

   • English Language Development (ELD) records (if applicable)

These documents can be scanned and emailed to csdregistration@centennialsd.org or copies can be delivered to The Centennial School District Registrar, 48 Swan Way, Warminster, PA 18974.

For families that do not have access to a computer, need technical or translation support, the Centennial School District is providing in-person registration events on the following nights. If you are in need of support, but cannot attend one of the dates/times listed below please contact the Registrar at 215-441-6000 x 11046, or email csdregistration@centennialsd.org, or click here to email CSD Registration.

If you plan to attend an in person event, you will need the following:

1. Email address & password (if you already have one, if not we will help you create one)

2. Skyward username & password (if you already have one, if not we will help you create one)

3. ALL of the REQUIRED documents listed in the "Online Registration" section above (if you are unable to make copies, we will scan the document for you)
